- The insurance policy or a transcript thereof.
- Application for Death Benefit： The insured’s death certificate and certificate of cancelled household registration, certified by a Taiwan representative office abroad or its authorized agent. If there should be no such certificate of cancelled household registration available from the insured’s national government, it shall be exempted.
- Application for Total Permanent Disability Benefit： A disability diagnosis, certified by a Taiwan representative office abroad or its authorized agent.
- Proof of the beneficiary's identity.
