- 【Application for Death Benefit】
- The insured’s death certificate and certificate of cancelled household registration, certified by a Taiwan representative office abroad or its authorized agent. If there should be no such certificate of cancelled household registration available from the insured’s national government, it shall be exempted.
- 【Application for Total Permanent Disability Benefit】
- A disability diagnosis, certified by a Taiwan representative office abroad or its authorized agent.
